We are your Patient Centered Medical Home with a focus on you. We want to partner with you for care that is truly “all about you, all around you.”  What does that mean to you?

Comprehensive Care – We have a team of health professionals to meet your medical needs. Our team consists of physicians, physician assistants, nurse practitioners, care managers, nurses, phlebotomists, psychologists, and other healthcare professionals, to meet your patient needs.

Patient-Centered – A Medical Home is a trusting partnership between a health care team led by your doctor and you – the informed patient. Tell us what you know about your health, illnesses, medications, needs and concerns. Take part in planning your care and follow the agreed-upon care plan.

Coordinated Care – We will work with other physicians, specialists, or care providers to assist in providing care with open communication and direct coordination of care.

Accessible Services – We offer access to an on-call physician 24 hours a day, 7 days a week. Additionally, we offer extended hours on Wednesdays (at Health Park) and Saturdays (at East Paris). The patient portal allows you to communicate with your physician at your convenience.  We strive to assist in managing your acute illness, as well as chronic diseases, and give advice to help you stay healthy.

Quality and Safety – We participate with industry performance measurements and review processes to ensure we are responding to patient experience and satisfaction, as well as achieving improved health for our patients.
